    indeed we need to activate the form after attesting it from COC.

    in jeddah, the activation counter is in the main branch of COC (Jeddah chamber of commerce building, near Tariq bin zeyad roundabout on al-andulus street and king abdullah road https://plus.google.com/107650387755680481363/about?hl=en)

    they also attest the application so don't need to go to any other office of COC in jeddah for family visit visa.
